Ultimate Care Services Ltd will provide a 24-hour care and support service enabling people to be independent and empowered so that they can live quality and independently in their own home. Families and carers will be offered home intervention care and support with the view of enhancing service user’s independence, health and well-beings.
Service users will receive a person-centred plans approach with realistic goals to enable maximum potential to be reached. The service objective is to ensure service user feels happy, secured and safe in their own home.
We are committed to providing support which is both people centred, and outcomes based. This means we take the time to get to know the capabilities, aspirations and challenges of each of our customers and help them work toward realistic and achievable goals. Fundamental to this approach is what we call Activity Based Care. This involves encouraging people to stay fit and active and do as many daily tasks for themselves as they are capable of doing. We often use creative methods to find new approaches and overcome obstacles when one of our service users is having difficulty. This helps each individual maintain their confidence and independence for as long as possible.
